Carpets, rugs, clothes, cushions, and bedding were hung over a clothesline or railing and the dust and dirt was beaten out of them. Typically made of wood, <a href="http://en.wikipedia.org/wiki/Rattan" title="Rattan">rattan</a>, cane, wicker, spring steel or coiled wire, antique rug beaters have become very collectible. Modern mass-production versions can also be in plastic or wire.</p> <h2><span class="editsection"></span> <span class="mw-headline" id="Uses">Uses</span></h2> <p>In the <a href="http://en.wikipedia.org/wiki/Netherlands" title="Netherlands">Netherlands</a> and parts of <a href="http://en.wikipedia.org/wiki/Belgium" title="Belgium">Belgium</a> the carpet beater was a common tool for mothers to discipline their children, by making them bend over and spanking them on their behinds, leaving a distinctive pattern on the child's (bare) buttocks. Like the actual beating of the rugs these punishments mostly took place in the backyards, turning the beating into a somewhat public event. This form of punishment nowadays is almost extinct in The Netherlands but was very common up until the mid-1980s; since then the use of instruments such as wooden spoons and carpet beaters in spanking has rapidly grown out of fashion.</p> <p>This 'secondary use' earned the carpet beater a special place in Dutch folklore, as a symbol for good housecleaning, conservative family values and childrearing, as well as a symbol for the dominant position of the housemother in traditional Dutch families.<sup class="Template-Fact" style="white-space: nowrap;">[<i><a href="http://en.wikipedia.org/wiki/Wikipedia:Citation_needed" title="Wikipedia:Citation needed"><span title="This claim needs references to reliable sources from June 2011">citation needed</span></a></i>]</sup></p> <p>Its use in cleaning has been largely replaced since the 1950s by the <a href="http://en.wikipedia.org/wiki/Carpet_sweeper" title="Carpet sweeper">carpet sweeper</a> and then the <a href="http://en.wikipedia.org/wiki/Vacuum_cleaner" title="Vacuum cleaner">vacuum cleaner</a>. Acceptance is the hardest part of the divorce recovery process. Acceptance requires total honesty, courage and the willingness to let go of the life that we had...a life that no longer exists. Without that acceptance, we cannot move forward and create a new life.<br /><br />How does one learn acceptance? Although it takes time and a good deal of inner work, it can be done. Here is a step-by-step guide to move you towards acceptance:<br /><br />1) It’s about you, not them.<br /><br />One of the most powerful lessons in life is the knowledge that we have control over one person and one person only…ourselves. If you are looking outside of yourself to move forward, you won’t. We can’t change anyone but ourselves. We have power over no one except ourselves. It is when we turn inward and do the work on ourselves that we will be able to effect dramatic and positive changes in our lives.<br /><br />Being a victim means giving away all control and power. If I blame someone else for my situation, then I am powerless to do anything about it as I have chosen to absolve myself of any responsibility.<br /><br />We can create changes that will make out lives better but not until we stop trying to change our ex or our current reality and we realize that it’s about us, not them.<br /><br />2) Get support.<br /><br />If you think you can do this all by yourself you may be in for a big surprise. Research consistently shows that getting support in any challenging endeavor leads to more success. Whether you choose a divorce support group, a therapist, a member of the clergy or a Life Coach, just do it.<br /><br />If you are one of the those people who think that you have to handle life’s challenges on your own because somehow you equate support with weakness, get over it! Getting support is a sign of intelligence as far as I’m concerned as well as an indication that you really are serious about moving onward in life.<br /><br />3) First, you must get through the initial stages of loss that includes denial, grief, anger, depression and whatever else you might be feeling early in the divorce process.<br /><br />These emotions are all natural and necessary states that we need to experience. They are the norm versus the exception. Each one of these feelings needs to be embraced and experienced fully. There must be an ending before a new beginning.<br /><br />There is a difference between fully experiencing an emotional stage and getting stuck in it. Beware excessive self-pity and real depression. Here is where support becomes important to your well-being and improvement.<br /><br />4) Distinguish between facts and interpretations.<br /><br />I cannot stress the importance of this step enough. People get stuck when they cannot face the facts and prefer to believe that their personal interpretations are reality.<br /><br />You might be familiar with the exercise of the picture that has a hidden image within it. Ten people may come up with ten different interpretations of the picture. Some people will see the hidden image immediately and others will never see it until it is pointed out to them. Either way, the hidden picture exists. It is a fact.<br /><br />You may feel that you have been mentally abused and yet your partner may feel that you are the one that is abusive. He said, she said. Probably a counselor will see a totally different picture altogether. You know, there’s your side, his side and then the truth.<br /><br />Once you are truthful with yourself and can see the facts versus the drama or story of your divorce, you will be on your way to acceptance.<br /><br />5) Be brutally honest and take responsibility for your marriage, divorce and life.<br /><br />Those of us who can be totally honest with ourselves will receive the gift of a deep awareness of who and what we are along with the ability to accept our lives as they are without looking to blame someone else. Being honest allows us to see things that hadn’t existed for us before. The truth will indeed set you free. By setting aside our egos, we can look at our life for what it actually is versus a story about our divorce.<br /><br />Once we have been honest and have embraced all the facts about our divorce, we are free to accept full responsibility for our lives. Responsibility is power and the freedom to choose what we want next in life. If we cannot take responsibility, we remain victims and victims absolve themselves of both their responsibility and therefore the power to control their own lives.<br /><br />6) Learn the difference between what is and what you think should be.<br /><br />If we are living in a netherland of what we think should be, we are completely cut off from reality or ‘what is’. If you think that you should not have to be experiencing divorce, then you cannot accept what is…that you are indeed getting divorced. You live in a world of your own.<br /><br />We all create a list of should be’s that keep us stuck in the status quo: I should be happier, I should be getting more support, I shouldn’t have to work, and I should still be married. By concentrating on what we should be, we ignore what actually exists for us and remain stuck.<br /><br />I think we should live in a world where peace is the predominant ethic but we don’t live in that world. That’s a dream I have. By acknowledging the world as it truly exists, I can make choices as to how I will live my life and also how to address the problems that do exist.<br /><br />7) Consider the emotional wounds that you brought to the marriage.<br /><br />Your ex may complain that you were not a warm person. I doubt that it was your marriage that created a cold person, if indeed that is what you are. We bring ourselves into our marriages and the parts of us that show up and create issues are the parts of us that we haven’t addressed yet. They are emotional wounds from somewhere in our past and they have a tendency to pop-up in our close relationships or when we are faced with challenging times.<br /><br />Now is your chance to address those wounds and heal them so that you do not repeat your so-called mistakes again. Use your divorce as a catalyst to go inside and heal yourself.<br /><br />8) Release toxic emotions.<br /><br />Get rid of the debilitating toxic emotions that you are carrying around. Picture them as heavy baggage that keeps you stuck in your misery and produces a broken back. Anger, bitterness, hatred, resentment, rage…these are all toxic emotions that will harm you far more than your ex. You are the one who pays the price. You need to work through them and then release them because they will weigh you down for the rest of your life if you allow it.<br /><br />Once you have done the work of truth versus interpretations and what is versus what should be, you will find it much easier to give up your anger and resentment. They do not serve you and you are learning to give away anything that does not serve you well.<br /><br />9) Learn forgiveness for yourself and your mate.<br /><br />You might not be able to practice forgiveness in the early stages of the journey to recovery but if you go through these other steps, you will be able to forigive your ex and more importantly, yourself. Forgiveness takes a big load off your shoulders. It releases energy that can be used for positive things.<br /><br />Forgiveness does not necessarily mean you condone bad behavior, it simply means you forgive. If we separate the person from the behavior it becomes easier to forgive. You know that just because you sometimes say mean things it does not mean you are a bad person. Just a lapse in judgment. We are not necessarily our behavior. We know all the subconscious motivations that exist within every individual. If we look at the inner child within a person, forgiveness is a given.<br /><br />10) Make conscious decisions; utilize free choice.<br /><br />When you do the inner work of divorce recovery, you tend to attend to many things that have been left unresolved for years. You become more conscious of your actions and your choices. You become aware of the subconscious and how it can run your life. When you learn to observe the constant mind chatter that goes on inside your heads, you learn that the mind chatter is not us, its just chatter.<br /><br />Making conscious decisions based in free choice means that we are not letting our mind chatter, our past, our emotional wounds or our interpretations of reality run the show. We take control of our lives. Conscious living allows for incredible freedom and the ability to create extraordinary changes.<br /><br />And your bonus tip:<br /><br />11) Find the gifts of your divorce.<br /><br />Everything that occurs in our lives and everything that we are (warts and all) has a hidden gift. If you speak to someone who has survived divorce and has gone on to create a and vibrant life based upon their own passions and values, they will certainly tell you that their divorce was the best thing that happened to them. That may not be true for you but there is a gift waiting for you to find. My ex likes to say that he is responsible for my new career and to a certain extent he has played a part. Oftentimes it takes a good whack on the head to awaken us to life’s possibilities and our own happiness.<br /><br />It’s the old adage: Every cloud has a silver lining. It is true. Search for the gifts of your divorce and it becomes yet another step toward a successful recovery from the trauma of divorce.<br /><br />Successful divorce recovery takes inner work. Much like a flower, the work that has takes place underneath the surface of the ground, invisible to the human eye, is the crucial aspect. Without that subterranean work, there would be no flower. The reward of the flower is dependent upon the inner work of the seed and the root system. It is the same with humans. room still soiling your underwear slightly and your sex life is, well ... it's not.

<br />Natural prostate remedies or man-made prostate drugs? Which to choose? Let's take a look at the drugs, what they do, their potential side effects, and how long they take to work.
<br />
<br />There are two types of prostate drugs for treating your prostate problem;
<br />
<br />1) 5-alpha reductase inhibitors, (commonly known as finasteride) these actually shrink the prostate in volume, and there are a few; Proscar, Propecia, Fincar, Finpecia, Finax, Finast, Finara, Finalo, Prosteride, Gefina.
<br />
<br />2) Alpha blockers, they relax the muscles at the neck of the bladder and make urination easier. Here are some that are used, Cardura, Flomax, Hytrin, Regitine, Minipress, and Uroxatral .
<br />
<br />However, there are large quantity of side-effects that men experience with these prostate drugs, and one of the things you'll find if you surf the forums on the internet, is that many men experience problems that are irreversible. You just don't know what's going to happen until it happens. To be fair, some of these side effects of these prostate drugs will decrease over time, but in other cases they won't. To be honest, not my cup of tea.
<br />
<br />So here's the list of side effects of these prostate drugs;
<br />
<br />- Cardura - COMMON - Back pain, diarrhea, dizziness, drowsiness, dry mouth, feeling of a whirling motion, flu-like symptoms, headache, indigestion, lightheadedness, nasal congestion, nausea, pain, stomach pain, swelling, tiredness.
<br />
<br />- Floxmax - COMMON - Dizziness, unusual weakness, drowsiness, trouble sleeping, or runny nose may occur.
<br />
<br />SERIOUS - fainting, vision changes, sexual problems (difficulty ejaculating, decreased interest in sex), fastheartbeat. in the unlikely event you have a painful, prolonged erection lasting 4 hours or more, stop using this drug and seek immediate medical attention or permanent problems could occur. Symptoms of a serious allergic reaction include: rash, itching, swelling, severe dizziness, trouble breathing.
<br />
<br />- Hytrin - COMMON; Difficult or labored breathing, dizziness, headaches, heart palpitations, light-headedness upon standing, nausea, pain in the arms and legs, sleepiness, stuffy nose, swollen wrists and ankles, weakness.
<br />
<br />LESS COMMON - Anxiety, back pain, blurred vision, bronchitis, conjunctivitis (inflamed eyes), constipation, decreased sex drive, depression, diarrhea, dimmed vision, dry mouth, facial swelling, fainting, fever, flu or cold symptoms (cough, sore throat,
<br />
<br />runny nose), fluid retention, frequent urination, gas, gout, impotence, inability to hold urine, increased heart rate,indigestion, inflamed sinuses, insomnia, irregular heartbeat, itching, joint pain and inflammation, low blood pressure, muscle aches, nasal inflammation, nervousness, nosebleed, numbness or tingling, painful lasting erection, pain in the abdomen, chest, neck, or shoulder, rash, ringing in the ears, severe allergic reaction, sweating, urinary tract infection, vertigo, vision changes, vomiting, weight gain.

<br />- Regitine - Acute and prolonged hypotensive episodes, tachycardia, and cardiac arrhythmias (irregular heartbeat). In addition, weakness, dizziness, flushing, orthostatic hypotension, nasal stuffiness, nausea, vomiting, and diarrhea may occur.
<br />
<br />- Minipress - Dizziness, drowsiness, head pain, visible water rretention, heart throbbing or pounding, dry mouth, low energy, nervous, not feeling well. SERIOUS; angina, continued painful erection, trouble breathing, feel like throwing up, frequent urination
<br />
<br />- Uroxatral - Dizziness/lightheadedness or headache may occur, decreased sexual ability occurs, unusual fatigue/weakness, joint pain, trouble breathing, stomach/abdominal pain, back pain. SERIOUS: unusually fast heartbeat, chest pain, a painful, prolonged erection.
<br />
<br />- Proscar, Propecia etc... all contain Finasteride - COMMON - Feeling like you might pass out, swelling in your hands or feet, pain in the testicles, impotence, loss of interest in sex, or trouble having an orgasm, abnormal ejaculation, swelling or tenderness in your breasts, dizziness, weakness, headache, runny nose, skin rash.
<br />
<br />SERIOUS; hives; difficulty breathing; swelling of your face, lips, tongue, or throat. Call your doctor at once if you notice any breast lumps, pain, nipple discharge, or other breast changes. These may be signs of male breast cancer.
<br />
<br />If you are using any of these drugs and have experienced ANY of these side effects, contact your doctor. If it commons under the SERIOUS OR LESS COMMON title, then seek urgent medical attention.
<br />
<br />Okay, not something to look forward to ... but don't despair there are other ways which I talk about in my other articles.

<br />Here's HOW LONG these prostate drugs take to reduce your symptoms;
<br />
<br />Cardura, Flomax and Hytrin etc... results are meant to be noticed within 1-2 weeks.
<br />
<br />Proscar and Propecia etc... It can take a long time for the drug to show improvement in symptoms - 6 months to a year for complete results.
<br />
<br />General Information;
<br />
<br />- Hytrin - designed as a long-term medication, studies have shown it to be more effective than Proscar, despite Proscar actually shrinking the prostate and it's more widely publicised use.
<br />
<br />- Proscar and Propecia have the good side effect of promoting hair growth (on your head, not on your back)
<br />
<br />- Hytrin, Cardura are also used to treat hypertension (high blood pressure)
<br />
<br />- Cardura, Flomax and Hytrin etc... advise not to climb, stand up quickly or drive when starting out on your doses.

<br />- Proscar, Propecia etc... can damage a male fetus if handled by a pregnant female.

local Walgreens I noticed that they carry hundreds of different herbs that claim to do amazing things. <br />1. GINKGO BILOBA LEAF – I wanted to start with ginkgo because it is commonly thought of as an herb to help with memory and brain function. This is in fact correct, however only covers one scope of the herb's benefits. Ginkgo works by expanding the capacity of blood flow through your veins and arteries therefore that effect benefits many parts of the body from the increased oxygen supply to the brain, which allows better brain function, to fuller and harder erections as more blood is supplied to the penis.

<br />A few concerns with ginkgo are, because of the increased blood flow, those with bleeding disorders, especially those whose blood fails to coagulate on a wound, should take more precaution as more blood flow will slow the coagulation process. The second concern is for those who prefer ginkgo biloba seeds over the leaf. Uncooked ginkgo biloba seeds have shown some small levels of toxins, so if you prefer the seeds it is advised to cook them prior to ingestion. Finally, as with any new addition to your diet, some people have claimed a minor upset stomach and diarrhea.

<br />The recommended concentration is 24% flavones and 6% terpene lactones. The usual dosage is 120mg to 160mg per day.

<br />2. EPIMEDIUM LEAF EXTRACT – The next herb is epimedium leaf or commonly known as horny goat weed. The origins of epimedium is from goat herders who noticed that when their flock ate the weed their sexual activity was noticeably increased, thus the name. Discovery Channel did a study on horny goat weed. They found that it is the number one most prescribed solution to sexual dysfunction by professional doctors in China. Although little is known about how horny goat weed actually works, it has shown significant increases in libido and vitality in both men and women, restore sexual function in men, and help with discomfort during menopause in women.

<br />To this day there have not been any known side effects with horny goat weed, other than the typical minor upset stomach and diarrhea when something new is added to your diet.

<br />It is recommended that your intake of horny goat weed is concentrated enough to have at least 10% icariin. Generally 1000MG to 2000MG is a sufficient daily dosage.

<br />3. HAWTHORN BERRY – The hawthorn berry is another herb that is generally used to treat one concern, yet its benefits extend further. The hawthorn berry is primarily used to treat heart problems. It strengthens the heart allowing it to pump more blood. This, of course, translates into more blood flowing into your penis. Thus allowing for harder, fuller erections and increased pleasure from orgasm.

However most studies indicate that long term use (12 months or more) of saw palmetto also eased urination and increased sexual function significantly.

<br />One of the few things about saw palmetto berries is that although there have been many studies done on saw palmetto, there have not been any reported side effects, none even complained about upset stomach or diarrhea.
<br />
<br />320mg per day standardized to not less than 85% fatty acids is the recommended dosage for saw palmetto berries.

<br />5. CATUABA BARK EXTRACT – The catuaba is a Brazilian, primarily in the Amazon rainforest, tree whose bark has been used for thousands of years as an all around nervous system revitalizer. It restores your nervous system function bringing about increased brain function, reduced nervousness, better sleep and improved sexual function. This is the herb that has helped the 60, 70, 80 even 90 year olds in Brazil have a healthy sex life naturally.
<br />
<br />There are no known side effects with catuaba except the occasional upset stomach and diarrhea when it is first introduced to the diet. The main concern is that you are actually purchasing real catuaba bark, as there are several exporters who export the bark as catuaba, but it is in fact from the wrong tree. There are 2 forms of catuaba tree that supplies the actual bark that is medicinally known for its benefits and the scientific names for these two is Erythroxylum catuaba and Trichilia catigua.
<br />
<br />The standard daily dose is 1000mg to 2000mg. There has not been any standardization of the extract’s concentration.
<br />
<br />6. MUIRA PAUMA BARK EXTRACT – The muira pauma is another rainforest tree found in South America and Africa. This plant’s benefits also focus on the nervous system to treat various ailments such as paralysis, beri-beri, rheumatism, grippe, and cardiac and gastrointestinal weakness, baldness and sexual dysfunction, such as impotence, infertility and menstrual disturbances.
<br />
<br />There are no reported side effects from muira pauma except the occasional upset stomach and diarrhea when it is first introduced to the diet. It is also commonly exported as muira pauma bark extract from a similar tree whose bark does not have the same medical benefits, the scientific name for it is Liriosma ovate, so be sure that before you purchase muira pauma that it does not come from this tree.
<br />
<br />The recommended dosage is 1500mg daily and there are no concentration standards for the muira pauma bark extract.
<br />
<br />7. CUSCUTA SEED EXTRACT – The cuscata is commonly known as dodder plant and it is commonly used to increase sperm production and its fertility. New research has shown that it helps with treating impotence.
<br />
<br />There are rumors that cuscuta seed extract, when ingested in high quantities, caused lack of sleep. There are no case studies showing this effect as the studies done use standard dosages when administering it. There have also never been any claims of upset stomach or diarrhea with cuscuta seed extract.
<br />
<br />The recommended dosage is 10g to 15g boiled in 3 cups of water for 20 minutes, some have said to increase that dosage up to 25 grams of cuscuta seed extract in 3 cups of water each day. It is standardized to 2.5% flavonoids.
<br />
<br />8. ASIAN GINSENG – There are several kinds of ginseng available on the market today, but the one that shows the most sexual function benefit is the asian ginseng. It is commonly used to restore energy both physically and mentally and in sexual function it has a significant benefit in preventing premature ejaculation.

He was a cartoonish 5 foot 3, 65 kilo powerhouse with a barrel chest and black pony-tail mullet down to his arse. At his gym in Bellingen he would ensure that I learned and retained precise lifting technique during my first 12 months of training. Thank-you Kevin. Under his supervision I went on to become the Australian School-boys Bench Press Champion at the ripe old of age of 15 with a lift of 87.5 kilograms (192.5 pounds) at a HUGE (laughs) body-weight of 66 kgs (145 lbs). That was after only 6 months of training. Technique and intensity is EVERYTHING!<br /><br />Kevin sold the gym to a great man called Scott who became a very close friend of mine, a fantastic training partner and the catalyst for my career in Personal Training. He looked somewhat normal in comparison to Kevin sporting a natural, balanced, muscular bodybuilder’s physique. His basic training philosophy was to hit each muscle group from every angle with perfect form designating set and exercise volume in proportion to the size of the muscle group, i.e. back being the most complex and largest received the most exercises while biceps being one of the smallest received the least. I learned a lot from him. I pay very close attention to the body position and movement pattern adjusting fine components so that the exercise is totally optimised. I must annoy the hell out of my clients sometimes. It works though.<br /><br />WHY IS PERFECT EXERCISE TECHNIQUE SO IMPORTANT?<br /><br />Reduced Risk Of Injury…<br /><br />If you are to remain injury-free throughout your training career you must practise safe, tight, controlled lifting technique for every single repetition. If the movement is as close to biomechanical perfection as possible you will receive maximum stimulation with minimum risk. Injuries will halt your progress every time and can be very serious. Any chance to avoid them should be harnessed, implemented and retained.<br /><br />Decreased ‘Cheating’ And Involvement Of Unrelated Muscle Groups…<br /><br />If your form is sloppy there is a good chance that you are not hitting the target muscle as completely as you could be. As soon as your technique declines other muscle groups come into play assisting during the lift, often in an unsafe manner. A classic one is allowing your torso to swing during a bicep curl using your lower back, glutes and hamstrings to get the weight up. There is not only the danger of hurting yourself there is also the probability that you are not taking the target muscles to failure. Keep the form tight and you will stimulate the muscle safely and entirely.<br /><br />Decreased Involvement And Fatigue From Ancillary Muscle Groups…<br /><br />Related to the last point the execution of a perfect repetition every time will ensure the target muscle is isolated and fully fatigued before the ancillary muscles (assisting muscles) give out. To make this more clear I will describe two examples where this is a common occurrence. Example 1: when you perform a back exercise if you do not focus on engaging and isolating the targeted muscles of the back then the forearms and biceps will crash and burn before the back is fully worked. Example 2: when you perform a chest exercise if you do not focus on isolating the pectorals during the movement then the triceps and anterior deltoids will give out first impeding the progress of the chest. It is easy for ancillary muscles to come into play during an exercise if your form is sloppy because they are there to assist the movement. But because they are smaller and weaker if you do not make sure that the target muscle is completely fatigued first they will always give out holding backyour progress.<br /><br />Increased Motor Neuron Innervation And Muscle Fiber Activation…<br /><br />If you isolate the target muscle group and contract it slowly and intensely through the entire movement you will activate the greatest amount of muscle fibers. A safe and full range of motion (ROM) will involve muscle fibers from the origin (initial attachment) right across to the insertion (final attachment) of the muscle. A slow, intense, precise movement during the entire concentric contraction (muscle shortening) and eccentric contraction (muscle lengthening) will innervate more motor neurons ‘firing’ more muscle fibers. This will facilitate a maximum activation of the muscle fibers and total muscle contraction maximizing strength and muscle gain.<br /><br />Improved Mind-Muscle Connection…<br /><br />The mind-muscle connection is your ability to feel the target muscle contract completely and under total control during the execution of an exercise. Usually a muscle group that you find difficult to feel and to develop is one that you have a poor mind-muscle connection with. Consistent, strict form on all of your repetitions will help you gain control of these stubborn body-parts. The mind-muscle connection improves when the path from your motor cortex (the control centre of your brain for movement) to the working muscle becomes more efficient.<br /><br />Improved Focus, Discipline, Intensity And Quality Of Training…<br /><br />This level of precision for every single repetition that you perform during your workout demands a massive amount discipline, will-power and intensity. You control the movement of every exercise and apparatus as though it were part of your body. Be patient and apply it to all exercises performed until it becomes second nature. By this stage it will feel awkward to train with sloppy technique and you won’t miss it because you will become stronger and more muscular and injury-free.<br /><br />COMPONENTS OF A REPETITION<br /><br />1. Starting Point – where the body is in the appropriate position to begin the Repetition.<br /><br />2. Concentric Contraction – where the muscle shortens as the weight is lifted through the predetermined Range of Motion (ROM). Also called ‘The Positive Phase’.<br /><br />3. Mid Point – the point of greatest contraction at the top of the Repetition where you pause briefly before releasing the weight.<br /><br />4. Eccentric Contraction – where the muscle lengthens as the weight is released to the Finishing Point. Also called ‘The Negative Phase’.<br /><br />5. If you forget how to do something go back to the site and look it up.<br /><br />Repetition Speed…<br /><br />The repetition speed should for muscle growth and strength in bodybuilding, shaping and toning and for general health should be medium (1-2 seconds) during the Concentric Contraction (where the weight is lifted) and slow (2-5 seconds) during the Eccentric Contraction (where the weight is released). The exact seconds required for each phase of the repetition depends on the size of the range of motion (ROM) of each individual exercise. Example; Calf exercises have a smaller ROM so the time required to perform the repetitions slowly would be reduced. Squats, on the other hand, have a large ROM so the time required to perform the repetitions slowly would be increased. Performing exercise with speed is not without merit and it is prevalent in functional training, power-training, and sport-specific training but there is an increased risk of tissue injury with the sudden force exerted when lifting. There is also greater focus on the tendons, ligaments, bones and joints rather than the muscles.<br /><br />Ancillary Muscles And Stabilizers…<br /><br />You cannot perform an exercise safely or effectively unless your body is positioned in accordance with the guidelines for that exercise. This means that once you are set your ancillary muscles (the direct assisting muscles for the movement) and the stabilizers (the muscles that hold the rest of the body in the correct place) must be engaged so that a perfect repetition is performed every time. This will make your body safe, strong and functional.<br /><br />Abdominals And Core Muscles…<br /><br />It doesn’t matter which exercise you are performing your mid-section must be switched on for total maximum gains. The abdominals and the core inner muscles of the spine are the superstructure of the body which holds everything together under the stresses of exercise. They lock you safely into the correct position and maintain it during the set. Engaging them properly will stabilise the body, increase your total strength, increase the strength of the abdominals and core muscles themselves, protect the spine and facilitate the implementation of perfect repetitions for all exercises. * To use your mid-section properly during exercise pull your lower abs in towards the spine, pull in the obliques, contract the entire abdominal complex and lift the chest *<br /><br />Symmetry And Precision Of Movement…<br /><br />To be certain that both sides of your body develop evenly you must ensure that each exercise is executed symmetrically. Human bodies are never built with faultless balance and the rigours of life make that deviation from perfect symmetry even more profound. To diminish the impact of this continuing you need to train in front of a mirror as much as possible or utilise the constant critical eye of a training partner. This will allow you to supervise and fine-tune your exercise technique at all times reducing imbalances of strength, muscularity and functionality.<br /><br />Range Of Motion…<br /><br />Most of the time you need to use a full range of motion (ROM) to stimulate the all of the muscle fibers from the origin, across the belly (the middle bulk), to the insertion of the muscle. If you cheat and continuously use partial repetitions you will lose flexibility and miss out on muscle and strength gains from the outer angles. There is a limit, however, to how large the ROM should be. It is individual to each exercise and should be large enough for maximum muscle fiber stimulation without causing any joint or tissue injury from over-extension. There are techniques and set sequences that are based on partial repetitions but they have their own strict technique and should be used sparingly for shock tactics and variety.<br /><br />Breathing…<br /><br />You need to breathe properly during each repetition to maintain a continuous delivery of oxygen around the body to the working muscles. It will also reduce the build-up of carbon-dioxide. This will give you the energy to power through the set and decrease the onset of blood lactate accumulation (OBLA – that awful burn from lactic acid which is caused from glycogen being burnt for energy without the presence of oxygen). Breathing properly also takes the pressure off your cardio-vascular system facilitating normal function under conditions of high stress from intense exercise. It is important to keep the blood pressure down and unnecessary strain off the body as no-one is unbreakable. Another advantage of full, rhythmic breathing is that it will pace the tempo of your repetitions. You can’t take slow, full breaths if your reps are fast and messy.<br /><br />BASIC BREATHING TECHNIQUE DURING A REPETITION<br /><br />1. Take a slow, deep intake of breath as the weight is released (when the target muscle stretches and lengthens during the eccentric contraction).<br /><br />2. Breath out more forcefully as the weight is lifted (when the target muscle engages and shortens during the concentric contraction)<br /><br />3. There are alternate breathing techniques but this one is simple and safe so remember: breathe in when the weight is released and breathe out when the weight is lifted.<br /><br />INTENSITY<br /><br />Finally we come to my favourite word; Intensity. To cause the greatest amount of muscle fiber damage and to induce growth producing chemistry within the body you need to push each set to absolute failure or beyond. By that I mean that you make sure you grind out each repetition with perfect technique until it is not possible to perform another without losing your form. Say the mother of a 22-year old student that you have treated requests to see her daughter’s medical records. The Bursar’s office confirms that the student is listed as a dependent for tax purposes. There seems to be no urgent reason for such a release and the student does not wish to give her mother access. How would you protect the privacy of her information?<br /><br />Situations such as this one that require knowledge of privacy laws to resolve successfully are all too common in the average student health center, yet the acronyms HIPAA and FERPA tend to strike fear into the hearts of the staunchest of college health professionals. So much has been written anecdotally on the subject of how complicated and unspecific these laws are that some may be surprised to find that according to legal professionals, the intersections between the laws are generally clear-cut. This article aims to explain which laws apply to you and what you can do to avoid the headaches that ensue from a conflict between your principles as a care provider and the law.<br /><br />Six golden rules of privacy law<br /><br />* FERPA never applies to non-students<br />* FERPA only applies when the student’s medical records are released<br />* HIPAA doesn’t apply to records covered by FERPA or to student “treatment records”<br />* Even if you treat non-students, you’re not bound by HIPAA unless you perform electronic transactions.<br />* Student health and counseling centers that do perform electronic transactions for non-students only have to abide by HIPAA for those transactions.<br />* State laws are applicable whether or not other federal laws apply<br /><br />This is how these rules break down.<br /><br />RULE 1: FERPA never applies to non-students<br /><br />RULE 2: FERPA only applies when the student’s medical records are released<br /><br />The Family Educational Rights and Privacy Act (FERPA) is the older of the two federal privacy laws. Enacted in 1974, one aspect of its governance is the privacy of educational records. There is a popular myth circulating that student medical records fall under the FERPA’s umbrella term “educational records”. In fact, FERPA specifically excludes the treatment records of students in higher education from its definition of educational records (see USC 20, 1232g for a complete definition). It also excludes employees of an educational institution if they are not students. FERPA does come into play, but only if the records are released to someone outside the health center, whether that is the student, their parents, their professors, or another health provider outside the university, at which point they become “educational records” rather than treatment records.<br /><br />It is important to note that it is not the request for the release that brings FERPA into effect. Many student health professionals believe that if a request to see the records is made that is in accordance with FERPA guidelines, they have to release them or be in violation of FERPA. Not so, says Kristine Dunne, BA, EdM, JD, an associate at the <st1:place st="on"><st1:city st="on">Washington</st1:City>, <st1:state st="on">D.C.</st1:State></st1:place> office of law firm Arent Fox, LLC.<br /><br />“It's the release of the records that triggers FERPA,” she explains. “There are no rights extended under FERPA to those medical records until such time as they have been made available to someone other than the treating health professionals, at which point the FERPA protections of student records kick in.”<br /><br />Applying this to the example at the beginning of the article, if state law doesn’t require you to release the student’s unreleased medical records to her mother, you are under no legal obligation to do so without a court order. Similarly, even if you think a professor may have a “legitimate educational interest” in requesting a student’s unreleased medical records, you still don’t have to release them.<br /><br />FERPA is just one part of the puzzle, however. The Health Insurance Portability and Accountability Act (HIPAA) of 1996 is another relevant law that seeks to be the national privacy standard in health care. It was updated in 2003 to take into account the trend toward automation and electronic record-keeping. These privacy guidelines have been well publicized and generally uphold the kind of patient confidentiality that most health care providers are comfortable with and there has therefore been a widespread trend in health centers to apply these standards to student medical records, even if they are not legally required. It is important to realize, however, that while its principles of privacy and confidentiality are excellent, in most cases, compliance is not required by law.<br /><br />RULE 3: HIPAA doesn’t apply to records covered by FERPA or to student medical records which are made, maintained, or used only in connection with the provision of treatment to the student, and are not available to anyone other than persons providing such treatment.<br /><br />RULE 4: Even if you treat non-students, you’re not bound by HIPAA unless you transmit health care information in electronic form in connection with the submission of claims for payment.<br /><br />HIPAA’s definition of protected health information (PHI) specifically excludes education records covered by FERPA and the treatment records of students in higher education as defined above. Dunne explains that the goal of this exclusion is simplification.<br /><br />“If student medical records were subject to HIPAA, there would be two completely different schemes – up until the health center released the record, it would be governed by HIPAA, and when it had been released it would be governed by FERPA,” she says.<br /><br />This was apparently considered unworkable by Congress, hence the blanket exception that HIPAA makes for any kind of student medical records. However, many student health and counseling centers also treat non-students, and this is where it starts to get a little bit trickier. To be considered a “covered entity” (i.e., bound by HIPAA), your health center must electronically transmit health information in connection with a “HIPAA transaction”. More detailed information on what constitutes a HIPAA transaction can be found in this primer released by The American Council on Education, but essentially it is any administrative or financial task carried out in the course of health care that transmits PHI. If you don’t perform electronic transactions, you don’t have to comply with HIPAA.<br /><br />RULE 5: Student health and counseling centers that do perform electronic transactions for non-students only have to abide by HIPAA for those transactions.<br /><br />Usually, every transaction of “covered entities” has to be bound by HIPAA standards, even if they are not all electronic transactions. However, because of the intersection with FERPA, these health centers are able to be bound by HIPAA just for the non-student transactions.<br /><br />RULE 6: State laws are applicable whether or not other federal laws apply<br /><br />With all the fuss about HIPAA and FERPA, don’t forget about your state’s laws concerning privacy. In some cases, state laws are the only ones that will apply to student medical records, but even where HIPAA or FERPA apply, state law is still relevant. Despite the fact that HIPAA is a federal law, it bows to state law in those cases where state law is more stringent. Arent Fox Associate Richard Liner, BA, JD, MPH, elaborates:<br /><br />“HIPAA has an enormous pre-emption problem because it sets a floor and not a ceiling for health care privacy. Congress only established a minimum for protecting patient information. If a state’s laws or regulations are more stringent than HIPAA in their protection of patient health information, then covered entities must follow state requirements.”<br /><br />This may conjure up ideas of conflicting laws, but Arent Fox counsels that generally, state laws are more specific and will very rarely conflict directly with HIPAA or FERPA. If more than one law is applicable, generally the more stringent requirements will apply. When in doubt, consult counsel before taking action.<br /><br />Knowing the theory is one thing, but applying it can be a lot more complicated. FERPA requires the student to give written, dated permission before his or her student records information is released – even to other health care providers outside the university, which is a source of frustration for many. But the same information can be released, unauthorized, to school officials who have a “legitimate educational interest”. Similarly, FERPA allows unauthorized disclosure in an emergency, if it is “necessary to protect the health or safety of the student or other persons”. Dunne counsels to rely on common sense to interpret these terms, and to consult counsel early in the process. No law can specifically cover every eventuality; the burden of responsibility and interpretation must, through necessity, rest on the care provider.<br /><br />This responsibility weighs all the more heavy because schools are concerned about penalties for breaching FERPA. If the Family Policy Compliance Office (FPCO) found a pattern of violations of FERPA with no obvious attempts to follow the guidelines, it could result in a removal of federal funding. However, it is important to know that individuals cannot be prosecuted for a FERPA breach and individual students cannot sue for damages for such a breach. Schools should carefully develop, implement and maintain compliance oversight with regard to these important privacy laws in order to prevent unlawful release of student records. Likewise, if your school treats non-students, files electronic claims and is bound by HIPAA for those transactions, you should make sure that HIPAA protections are implemented, even though a HIPAA violation may not – for now – result in a fine being imposed. Liner explains:<br /><br />“In the vast majority of cases where there’s found to be a violation of HIPAA, there is what’s called an ‘administrative resolution’, which generally means the mistake wasn’t intentional and the organization voluntarily agrees to take appropriate remedial action.”<br /><br />No civil fines for violations of HIPAA have been imposed so far, although Liner warns that is likely to soon change.<br /><br />Although information on the triumvirate of privacy laws has always been available to those who know where to look for it, there is also a wealth of partial and incorrect information available on the Internet that has muddied the waters for those health professionals attempting to do a little research on the laws that apply to them. Dunne and Liner counsel that you should speak to a professional who knows the law in your state and the ins and outs of FERPA and HIPAA if you are worried about misinterpretation of the law. Even if you know the basics, state laws vary greatly and knowing the details of how the three laws intersect will allow you the greatest leeway to interpret them in a way that is consistent with your ethics.<br /><br />“It is complicated,” sympathizes Liner. “Talk to the privacy officer within the university, if there is one. People have differing opinions as to what acne really is. So, what exactly is acne? Do a couple of occasional but recurring zits qualify as a case of acne or do you have to have a lot of zits?<br /><br />Believe it or not, the answer is that occasional pimples or zits do not constitute a true case of acne. Although zits do seem to have the ability to appear almost instantaneously and at the most inopportune times like picture day, prom night, spelling bees, sporting events, dates, and special award ceremonies, they are not a true acne outbreak.<br /><br />The dictionary defines acne as "an inflammatory disease of the sebaceous glands, characterized by comedones and pimples, especially on the face, back, chest, and, in severe cases, by cysts and nodules resulting in scarring."<br /><br />The anatomical definition of sebaceous glands is: "small subcutaneous glands usually connected with hair follicles. The follicles secrete an oily semi-fluid matter, composed in great part of fat, which softens and lubricates the hair and skin."<br /><br />Real acne outbreaks are actually a disease of the skin. However, the great news is that acne is treatable. Although it can be very embarrassing, cause great emotional distress, and lower your personal self-esteem for a period of time, acne is not fatal.<br /><br />There are new and effective treatments being sought by researchers and great advances have been made in the treatment of acne in the last few years. Years ago, when a person had acne, they were pretty much stuck with the problem. There were very few treatments available and the medical profession didn't even consider acne a disease.<br /><br />It was long thought that acne was the direct result of a diet that was too high in fat and/or sweets. That is no longer the case. Acne is most often associated with puberty and the onset of pre-teen and teenage years but it can and does develop in adults as well. When acne does finally heal, there can be permanent scars left (from picking and popping) that are unsightly and cause patients to suffer long term emotional distress and low self- esteem.<br /><br />New and very effective skin resurfacing treatments have been developed over the last several years that have, if not completely removed acne scars, at least diminished their appearance and severity.<br /><br />Acne Fact or Fiction:<br /><br />You can hear a lot of tall tales about acne today, so let's take a look at the fact or fiction of acne. It is always better to be well armed with factual information so that you don't get fooled by the fictional facts that surround acne.<br /><br />Fictional Fact #1: Acne is caused by a lack of sexual activity.<br /><br />Factual Fact: Acne and sexual activity are two entirely separate issues. One has no bearing on the other. Hormones secreted during puberty and young adulthood does have a bearing on acne. They also have a bearing on sexual arousal and activity. However, acne has no bearing on sexual activity nor does sexual activity have any bearing on acne.<br /><br />Fictional Fact #2: People have acne because they are dirty.<br /><br />Factual Fact: Dirt has no part in acne. Dirt is dirt. Acne is acne. One has nothing to do with the other. Acne is a build up of oil, dead skin cells, and bacteria. Dirt isn't involved. Keeping the face clean can and will help to prevent clogged pores but dirt does not cause acne.<br /><br />Fictional Fact #3: Dermatologists can cure acne.<br /><br />Factual Fact: Dermatologists can TREAT acne. They can help to alleviate the symptoms and help to clear up the pimples, black-heads, and white heads. They can prescribe antibiotics and topical ointments, lotions, and creams that will help but there is no cure at this time for acne.<br /><br />Fictional Fact #4: Acne is simply a skin problem.<br /><br />Factual Fact: It's true that acne affects the skin but it can also affect the way a person sees himself or herself. Acne and the scarring it can leave behind may cause a sufferer to become depressed and develop low self-esteem, both of which can lead to larger and more complex life socialization problems. Acne sufferers need the loving support and reassurance from their family and friends.<br /><br />Acne and Stress:<br /><br />Can stress actually cause acne? There is ample evidence available to suggest that stress can most assuredly cause an acne breakout or make an existing breakout worse. Our bodies are highly developed chemical laboratories that produce all sorts of stuff.<br /><br />At puberty, our body begins to produce an abundance of male hormones and this happens in both boys and girls. This overproduction of male hormones can happen at other times in life besides puberty; for example, when a girl or woman starting or stops taking birth control pills.<br /><br />These male hormones cause the bodies sebaceous glands to shift into overdrive and begin producing sebum. The sebum then travels up hair follicles, clogs the pores and begins the acne development cycle. However, male hormones are not the only cause for the sebaceous glands to begin producing an overabundance of sebum. When we become extremely stressed or overly emotional, our bodies react by causing the adrenal glands to produce a substance known as Cortisol, which is released directly into the bloodstream. This is good news for the thousands of victims of prescription drug problems who end up needing drug detox.<br /><br />The majority of drug-related trips to emergency rooms and detox centers – not to mention sudden deaths and trips to the morgue – are caused by problems with prescription drugs. Hospitals, ERs and detox centers in recent years are seeing more dependencies, addictions, overdoses and life-threatening drug interactions as a result of prescription drugs than more traditional illicit street drugs like heroin, cocaine in its many forms, and perhaps methamphetamine.<br /><br />Most drug detox facilities now provide specialized medical drug detox services for a host of different prescription drugs. And although detox centers help people with problems caused by legitimate prescriptions, many of the most commonly abused prescription drugs have found their way into the illicit street drug pipeline. And virtually all of the popular prescription drugs now found in schoolyards, night clubs and on the streets require drug detox to safely get off them.<br /><br />The new prescription drug program, called ePrescription, is used by doctors right in their offices to access online information about a prescription drug, including generic alternatives, potentially harmful drug interactions, and potential allergic reactions. Patients already taking other prescription drugs often don’t remember what they’re taking or forget to tell the doctor about them – or the doctor doesn’t ask. But because the program lists all the patient’s medications, it helps avoid the serious side effects or even dependencies that lead to drug detox when the wrong drugs are combined. As initially tempting as all the treats in the case behind the sign looked, I had to laugh at the irony of the words on the sign. Nothing against Starbucks (I'm a huge fan of their chai tea mistos), because they are only a small part of a huge industry that sells false messages about food, but I have got to set the record straight. Low fat will not make you happy. Neither will a sweet indulgence. Temporarily, maybe. Long term, never.<br /><br />We are born into a world where we are given mixed messages about food, especially sugar. On the one hand: don't eat sweets -they are bad for you! They will make you fat! They will cause your teeth to rot! They will give you diabetes! And on the other hand: Eat sweets - they will make you happy! Or creative! Or inspired! Let's celebrate with cake! Here's a piece of candy, now please stop crying! Or here's a cookie because you were good! I love you - here's chocolate!<br /><br />Pretty powerful - and confusing - messaging that is infiltrated into our brains from practically the time we are born (and then compounded a hundred times over by advertising). The message is this: That although sugar might be bad for our health, it can make us oh, so happy and, without it, our lives might feel depleted, depressing, loveless and lonely.<br /><br />Although we all know that too much sugar is not good for us, many of my clients, readers, friends and people in the audience at my talks find this white substance (or another form of sugar, like alcohol, white flour, or other quickly absorbed carbohydrates from processed foods) addictive and compelling. Despite our best efforts at "willpower," almost everyone has experienced the "I NEED SOMETHING SWEET OR CRUNCHY OR ALCOHOLIC NOW" syndrome. . . that intense craving for some substance that will bring us comfort or relief or cheer. And the reason is: it works! It really does. . . that is, it works temporarily.<br /><br />Like a drug, sugar (and its partners in crime) literally affects our brain so that we feel immediately relieved, calm and happy. Problem is, this feeling does not last and we often find ourselves later feeling equally - or more - down, upset or irritable. No amount of willpower works when our brains are working against us.<br /><br />And here's another crazy thing: Many of us are so used to feeling irritable, anxious, mildly (or intensely) depressed, tired, unfocused, and full of cravings, that this state of being feels almost "normal." But let me assure you: this is not normal. When our brains and bodies are balanced, our normal state is to feel good. Energetic. Calm. Focused. Satisfied and fulfilled and nourished by our food and by our lives.<br /><br />Now I am not blaming sugar for every emotional or physiological problem we experience. Anxiety, depression, irritability, lack of focus and cravings are often multi-determined and complex. And I must admit, I am hesitant to even write about this because I don't want to perpetuate the "bad food/good food" mindset that you may have. But, as a therapist and as a person who struggled with some depression, anxiety and eating problems myself, I am convinced that sugar's affect on our brain chemistry must be understood if we are to break out of some of these painful cycles.<br /><br />Let's use my day yesterday and my initial descent into "sugar hell" as an example of what could - and often does - happen when we eat sugar. I started off my day as usual - some protein, fat and complex carbohydrates (a big dose of peanut butter on a sprouted wheat bagel), which kept me pretty full and satisfied until lunch. At that point, my husband made the kids grilled cheese which looked so good that I couldn't resist. This is not my typical lunch and, although filling physically, did not feel at all satisfying. I knew I needed more protein and some vegetables, but we were busy so I skipped it.<br /><br />I had an amazing slice of cake at my daughter's birthday party and enjoyed every bite of it, but afterwards, found myself nibbling at the snack food that was out. And nibbling. And more nibbling. (Still hungry from lunch, and triggered by the sugar in the cake). I really couldn't stop which, these days, is quite unusual for me. When we left the party and got home, I found myself searching cabinets. I munched on the popcorn my husband made for the kids. I noshed on some left over cake (just straightened out the edges several timesJ). I picked at some leftover Chinese food.<br /><br />I was in quite a state really, searching, physically full, but unsatisfied, and noticing my mood shifting from calm and stable to mildly irritable and jittery. And because I have been in "sugar hell" before and am quite familiar with what I need to do to get out of it, I was able to stop. It is in these moments that I ask myself what I need to do to "right" my body's chemistry. Although I was physically full, I knew I needed real fuel for my brain in order to get off the rollercoaster I was on. I knew, for me, that fuel had to come in the form of protein (and I wanted red meat) and fat. I ate a good meal, watched a funny movie with my husband, and got into bed, finally satiated, physically and emotionally.<br /><br />Had I not been in tune with my body, or had I not really tuned in and listened, this "sugar hell" (or cycle of craving and munching and dissatisfaction with elevating irritability and anxiety) could have continued for quite a while (and let me assure you, it has in the past - in fact, I used to LIVE in this place!). When we eat sugar, our blood sugar temporarily rises and we feel good, leading later to a drop in blood sugar levels and an intense craving for MORE. Sugar may also temporarily increase our serotonin levels (our "feel good" brain chemicals). But again, these feelings don't last and ultimately, the sugar and fake foods (like a drug) deplete our bodies and our brains so that over time, we feel less good (and feel like we NEED more sugar to feel better).<br /><br />Unfair, I know. And I am not suggesting we never eat sugar again or never feed it to our kids. We need to balance our psychology and emotional response (the word NEVER tends to throw us into a state of feeling deprived) with our biology and physiology (the physical effect of these foods on our body and our brain), so that we can find a way of relating to sugar (and its counterparts) effectively. So that we can become aware of the effects of these foods on our bodies and arm ourselves against intense and continual cycles of fatigue, irritability and cravings.<br /><br />Here are a few tips that may help either prevent "sugar hell" in you or your children, or help pull you out of them if you happen to find yourself there:<br /><br />1) TUNE IN to your body. Figure out what works for you (and your children) and what keeps your blood sugar levels and brain chemistry stable. Notice what foods trigger cravings, difficult behavior,fatigue or irritability, and minimize them.<br /><br />2) Notice what emotional states might trigger sugar cravings - loneliness? Stress? Boredom? Fatigue? Begin to address them with non-food solutions and get help if you need to.<br /><br />3) Most people benefit from some combination of protein, fats and high-quality carbohydrates at each meal and snack to stabilize blood sugar levels.<br /><br />4) If you are going to eat sugar or processed carbohydrates, arm yourself by eating protein and/or fat (a piece of cheese, some nuts, some sliced turkey, for example), beforehand.<br /><br />5) Do NOT skip meals - ever.<br /><br />6) Avoid fake sweeteners (like NutraSweet, saccharine, Splenda, etc), which just "whet" your appetite for more sweets.<br /><br />7) Avoid packaged foods that are labeled low fat. In fact, don't be afraid of fat at all - most unprocessed fats are GOOD for your body and your brain (more on this another time!).<br /><br />8) If you find yourself in the middle of a sugar binge or "sugar hell," stop and ask yourself: "what is the next thing I can do to shift this cycle?" Then DO IT.<br /><br />9) Don't beat yourself up - for anything. Especially for not having "enough willpower." Remember, your cravings are not about willpower, but about your body's way of trying to tell you that you are out of balance somehow.<br /><br />10) Stop thinking of sweets as a reward. Schmetzer, MD, Fellow of the American Psychotherapy Association, and Master Therapist <br /><br />A number of veterans from Operation Iraqi Freedom/Operation Enduring Freedom (OIF/OEF) are returning home with signs of combat-stress-related Post Traumatic Stress Disorder (PTSD). In a recent study, 16.6% of the soldiers met the screening criteria for PTSD. On average, they showed a significant increase in sick visits, missed workdays, severity of somatic symptoms, and poorer overall health (Hoge et al., 2007). In another study, the youngest age group, 18–24 years, was at greater risk compared with veterans 40 years of age or above. Diagnosis was made early (median of 13 days), and most of them were detected in primary care clinics (Seal et al., 2007). <br /><br />Upon return from the war zone, veterans frequently report intrusive thoughts, flashbacks, increased vigilance, avoidance of social situations, hyperarousal, and nightmares. Treatment involves integration of mental health, primary care, physical medicine, attention to substance abuse, and vocational services. The mental health portion involves an initial screening of the combat veteran for PTSD and other mental illnesses, followed by a full assessment. Both pharmacotherapy and psychotherapy (individual, couple, and group) are offered for treatment. <br /><br />From a pharmacological perspective, several studies have found the traditional anti-depressants effective in PTSD. Selective serotonin reuptake inhibitors (SSRIs), like sertraline (Zoloft®), paroxetine (Paxil®), and fluoxetine (Prozac®), have been studied extensively for PTSD, and sertraline and paroxetine have been approved by the Food and Drug Administration for PTSD. SSRIs have been found to be effective both in short-term trials and long-term maintenance treatment for relapse prevention (Asnis et al., 2004). However, earlier studies have focused mainly on PTSD secondary to interpersonal trauma in a civilian setting. In a multicenter study, venlafaxine extended release (Effexor XR®), a serotonin norepinephrine reuptake inhibitor, was found to improve both the re-experiencing and the avoidance symptoms of PTSD, but not hyperarousal. The drug was effective and well tolerated in both short-term and continuation treatment of PTSD (Davidson et al., 2006). In a small study, mirtazapine (Remeron) was found to be effective in both short-term and continuation treatment of combat-stress-related PTSD without any serious side effects (Kim et al., 2005). In addition, sedation from mirtazapine can even prove beneficial in improving sleep in PTSD. In a randomized trial comparing phenelzine (a monoamine oxidase inhibitor) and imipramine (a tricyclic antidepressant), both significantly reduced combat stress related PTSD symptoms (Kosten et al., 1991). Benzodiazepines are used in PTSD for panic attacks or anxiety states. They provide temporary relief but run the risk of tolerance and addiction. <br /><br />Veterans with PTSD find it hard both to fall asleep and to maintain sleep due to hyperarousal and vivid nightmares related to combat. Significant others often report that patients scream in their sleep and may even wake up soaked in sweat. Prasozin and clonidine both decrease the central nervous system’s noradrenergic activity. They have been found to be effective in decreasing hyperarousal symptoms and improving sleep (Boehnlein, 2007). Other drugs used for sleep are the benzodiazepine class of drugs, like temazepam, and non-benzodiazepines, like zolpidem (Ambien™) and ezopiclone (Lunesta™). However, caution must be taken regarding the habit-forming potential of these drugs (Bhagar and Schmetzer, 2006). <br /><br />The presence of psychotic symptoms in PTSD can further complicate the clinical picture. In one study, 20% of the 91 males with combat-stress-related PTSD were found to be suffering from hallucinations and delusions, and hyperarousal was positively associated with the occurrence of psychotic symptoms (Kastelan, 2007). In a small study, augmentation of SSRI with olanzapine (Zyprexa), an atypical antipsychotic, was effective in treating SSRI-resistant combat-related PTSD symptoms, especially sleep (Stein, 2002). In another study, monotherapy with typical or atypical antipsychotics, reduced both PTSD and psychotic symptoms, and antipsychotics seemed to offer another approach to treat the psychotic subtype of combat–related PTSD resistant to previous antidepressant therapy (Pivac, 2006). <br /><br />Overall, PTSD pharmacotherapy involves several drugs based on our experience with PTSD in general, but well-designed studies are needed to establish treatment guidelines specifically for combat-stress-related PTSD.Unknownnoreply@blogger.com0tag:blogger.com,1999:blog-597733352695945496.post-82194676479392423462008-03-26T05:56:00.000+07:002008-03-26T05:57:53.768+07:005 Tips For A Healthier, Stronger Core<p style="text-align: justify;">by: <b>Yuri Elkaim</b> <o:p></o:p></p><div style="text-align: justify;"> </div><p style="text-align: justify;" class="MsoNormal"><br />Core training has become the new “buzz” word over the last few years as more and more people have begun to realize its role in posture, spinal health, performance and overall aesthetics. What is less commonly known is that the core is anatomically defined as the region between the shoulders and knees, not simply the midsection. Most people view the core as simply being their abdominal muscles and, as such, miss out on a tremendous amount of value that other functional movements provide.<br /><br />When I refer to the core I allude to the inner and outer units of our body. The inner unit consists of smaller, more static stabilizing muscles such as the transverse abdominis, multifidus, and the pelvic floor and diaphragmatic musculature. The outer unit is comprised of larger phasic (or dynamic) muscles that generate movement such as the gluteals, latissimus dorsi, erector spinae group, biceps femoris, and peroneals. Aside from generating movement, these muscles work synergistically to provide much needed pelvic stability during motions such as walking, running, and so forth. Because all functional movements such as lunges, squats, step-ups, most stability ball movements, and many others revolve around the pelvis, they will offer tremendous core training effects when done with proper technique. The following are 5 tips you can use to train the inner and outer units of your core with maximal efficiency.<br /><br />TIP #1 – Brace your abdominals<br /><br />This is the first thing you should be aware of during any and every movement that you will ever perform. Abdominal bracing consists of three parts. First, draw in your belly button as if you were to put on a tight pair of pants. This will activate your transverse abdominis, the body’s waist belt muscle. Second, raise your pelvic floor by performing a kegel (contracting the pelvic floor muscles up as if you holding in a full bladder). This helps to increase intra-abdominal pressure which will aid in spinal stabilization. Third, lightly create tension in your abdominal muscles as if you were about to get punched in the stomach. This activates the internal and external oblique muscles. By performing all three of these actions you will ensure that your spine is well protected through all movements. Abdominal bracing should be initiated before and maintained through each and every exercise. With repetition, these muscles will remember their roles and tend to maintain a tighter constricted waistline, even without you being aware.<br /><br />Exercise: Belly tucks<br /><br />Lie face down on the floor with your forehead rested on the back of your hands. Inhale, pushing your belly button into the floor (“ballooning” your abdomen). Hold for 3 seconds. Exhale, drawing your belly button towards the ceiling (as if being pulled by a string), raise pelvic floor, and tighten your abdominal muscles. Hold for 3 seconds. Repeat 10 times.<br /><br />TIP #2 – Maintain a neutral spine<br /><br />A neutral spine is attained when your spine is kept in its natural alignment. This is best achieved by ensuring that your back is flat (with slight lumbar lordosis) during all movements, especially lifting. This, in conjunction with abdominal bracing, is essential to better support the spine during all movements. Let us take the example of picking up a heavier box from the floor. The last thing you want to do in the situation is reach down without bending knees and picking up the box…this is a sure way to throw your back out. What is instructed to do in such a case would be to squat down keeping your torso tall (ie. neutral spine), with your abdominals braced, and lift through your legs as opposed to extending up through your torso. If these lifting mechanics are not met, you place you spine at a greater risk of disc herniation, especially if the load is heavier.<br /><br />Exercise: Cat/Camel motions<br /><br />In a four point stance (hands and knees) take your spine through a series of cat and camel stretches. Meaning that you round out the back like a camel and then arch it out like a cat. Repeat 6 times. Once you have completed the 6 repetitions simply allow your back (spine) to relax. Wherever it feels most relaxed is your neutral spine!<br /><br />TIP #3 – Incorporate unilateral lifting<br /><br />Unilateral refers to the concept of carrying (or pushing) a load on only side of the body. For instance, walking to work holding your brief case in one hand. By doing so, the body automatically activates its contralateral (opposite) side to stabilize the torso and maintain good posture. Studies have shown that this type of lifting stimulates much greater core muscle activation compared to bilateral lifting (equal load on both sides).<br /><br />Exercise: Unilateral Lunge Walks<br /><br />Perform your lunge walks while holding a weight, equivalent to 10% of your body weight, in one hand. Perform 10 reps with the weight in one hand, and then switch. The key is to focus on keeping your torso upright and minimizing and lateral swaying.<br /><br />TIP # 4 – Use a Stability Ball<br /><br />Incorporating a stability ball into your workouts will make such a difference in your core strength and spinal health. Working on the stability ball offers several benefits such as increased balance, range of motion, co-ordination, and muscle activation. Simply by sitting on the ball, your core muscles fire to a much greater degree in order to stabilize your body. Any unstable surface for that matter will foster much greater core muscle activation as your body is constantly readjusting itself to maintain proper posture. This type of body awareness is known as proprioception, and is immensely beneficial for athletes of all endeavors, and even for people who want more balance while standing in the subway. Incorporate the aforementioned unilateral lifting and you get twice the benefit! Exercise:<br /><br />Exercise: Stability Ball 1-Arm DB Chest Press<br /><br />Position your body on the ball so that only the shoulders, neck and head are resting on it. With the feet shoulder width apart raise your hips so that your body is in one straight line (essentially forming a bench within your body). Squeeze your buttocks together as if holding a $1,000 bill between them. Next, with a DB in one arm, push it up and towards your body’s midline as if creating an arc like motion. Repeat 12 times and then switch arms. Ensure to keep your body and then ball as still as possible. Notice the muscle activation in the posterior side of the body especially in the glutes and lower back!<br /><br />TIP #5 – Incorporate multi-planar movements<br /><br />Our bodies rarely operate in one single plane (ie. front to back, side to side). Often, our movements require us to move through several different planes such as when walking and looking back over your shoulder. It is important to remember that all movement stems from the core, especially rotation based movements. As such, it is important to strengthen those core muscles accordingly to ensure movement efficiency and injury prevention. Did you know that 80% of our core musculature inserts on a diagonal. This means that our bodies are anatomically constructed for such multi-planar movements involving diagonal and rotational movements. Examples are throwing, kicking, and swinging a tennis racquet or golf club. I’ve seen clients improve their golf drive by 30 yards by simply incorporating these multi-planar movements into their routines.<br /><br />Exercise: Cable chop (from knees)<br /><br />Position yourself in the middle of the cable crossover machine. Face your body at a right angle from the cables. Starting from your knees, keeping your body upright and strong, reach over and across your body grabbing the handle (with both hands) on the highest setting. Keeping both arms straight “chop” the cable across your body from above your starting shoulder to the opposite hip. Return slowly and repeat 8 times on each side. Be sure to drive the movement from your obliques and not your arms.<br /><br />By incorporating these 5 core essentials, you can look forward to having firmer, stronger and more performant core muscles. Not only that but you will also be more efficient and stable in all your movements.
